2005 – 2006 Season Coach And Referee Meeting
January 9, 2006 6:30 pm at the Veterans Memorial Building,
Culver City, corner of Overland and Culver
Before the meeting: Enter all your information into the ayso-online system (go to www.ayso1p.org and click on authorized users); send an email to webmaster@ayso1p.org if you need log-in information
At the meeting, bring:
- Team roster plus two copies, signed by your regional commissioner
- AYSO registration forms (medical releases) for all players (original signatures from parent or legal guardian, preferred if blue ink)
- Copy birth certificate or other official evidence of age for all players
- Signed Kidzone pledge for each player (available on the national website at http://soccer.org/media/programs/pledge.pdf
- Completed line up cards - you do not need these if you have entered all your information into the ayso-online system
- - Must include all players on rosters
- - Numerical order
- - League – 3 cards
- - All star – 7 cards
